Abstract
75 year old male with multiple medical problem, had severe aortic stenosis, up on work up for TAVI we discover that he do have small annulus and low left main take off which make the case difficult for Medtronic Valve, we planned to do Edward valve with low implantation also for more accurate I did aortogram during valvuloplasty, left main was patent but surprisingly post valvuloplasty he develop 4+ aortic regurgitation and ST elevation, another aortogram confirm the presence of MID RCA 100% stenosis. The plan was to go after the valve which was deployed successfully then RCA was intubated with JR4, Export catheter showed presence of big thrombus in spite of ACT 230, the valve was seated appropriately and the RCA was TIMI III.
